About the Role
The Nurse Practitioner will assess, diagnose, and manage acute and chronic conditions while working alongside medical professionals to ensure high-quality patient outcomes.
Responsibilities
- Evaluate patient health through comprehensive assessments and medical histories
- Diagnose and treat acute and chronic illnesses independently and in collaboration with physicians
- Develop, implement, and adjust individualized care plans
- Order and interpret diagnostic tests including lab work and imaging studies
- Prescribe medications and manage therapeutic regimens
- Provide patient education on disease prevention and health promotion
- Monitor patient progress and modify treatment as needed
- Collaborate with specialists and healthcare providers for coordinated care
- Maintain accurate and timely medical documentation
- Ensure compliance with healthcare regulations and standards
- Participate in care coordination across multiple settings
- Support transitional care processes for improved patient outcomes
- Respond to urgent patient needs and medical changes
- Perform physical examinations and routine health screenings
- Assist in developing clinical protocols and quality improvement initiatives
- Engage in interdisciplinary team meetings and case reviews
- Advocate for patient needs within the healthcare system
- Utilize electronic health records for patient management
- Uphold patient confidentiality and ethical standards
- Stay current with clinical guidelines and medical advancements
