Responsibilities
- Offer clinical assistance to caregivers and patients by phone outside regular business hours
- Perform supervision calls with caregivers and evaluate patient care plans
- Address and provide guidance on clinical inquiries and issues that occur after hours
- Support consistent, high-quality patient care by delivering accurate advice and escalating when necessary
- Record all communications and share critical updates with relevant care team members
- Demonstrate professionalism and compassion during every interaction with patients and caregivers
