Manage patient care pathways from a distance as a US-licensed Registered Nurse in a role focused on clinical assessment, care planning, and care coordination. You'll evaluate patients across medical, functional, and psychosocial dimensions, then build and track individualized plans that support better health outcomes and efficient resource use.
Key Responsibilities
- Perform in-depth assessments covering clinical status, behavioral health, and social determinants of care
- Create and oversee care plans with defined, measurable objectives
- Coordinate services across providers, facilities, and community programs
- Spot gaps in care delivery and take action to address them
- Refer patients to long-term support or disease management programs when appropriate
- Document assessments, interventions, and follow-up activities accurately and promptly
- Join interdisciplinary team meetings to share updates and align on patient progress
- Guide patients and families through care plans, self-care techniques, and support resources
- Support smooth transitions between care settings, such as post-hospitalization
- Uphold HIPAA standards and adhere to regulatory and internal compliance requirements
- Contribute to audits, reporting tasks, and initiatives aimed at improving care quality
What You’ll Need
- Active US Registered Nurse (RN) license
- Strong written and verbal communication skills
- Ability to manage multiple cases efficiently with solid organizational habits
- Proficiency with Microsoft Office and electronic health record systems
- Reliable computer setup with a noise-canceling headset
- Second monitor to support multitasking and documentation efficiency
- Stable primary internet connection and a backup connection
Preferred Background
- Experience in case management, utilization review, or managed care settings

Work Environment
This is a fully remote position requiring availability during US Pacific Standard Time, Monday through Friday, 9 AM to 6 PM.
