Responsibilities
- Interpret billing, reimbursement, and third-party payer requirements related to pharmacy services and products.
- Understand and navigate electronic health record systems used at CarepathRx.
- Support physicians and healthcare providers in designing and executing patient care plans to ensure safe and effective medication use.
- Perform initial and ongoing assessments of patient medication and medical history to detect actual or potential drug-related issues.
- Collaborate with physicians and clinical staff to define treatment goals and establish monitoring criteria for drug therapy safety and effectiveness.
- Evaluate drug therapy outcomes by analyzing lab results, clinical findings, and other relevant patient data.
- Report findings from therapeutic monitoring to physicians, nurses, and other clinical staff, offering recommendations to improve treatment outcomes.
- Provide expert advice and respond to inquiries from patients, families, and healthcare professionals about medications and their use.
- Ensure accurate and complete documentation of all clinical interactions and communications in patient records.
- Demonstrate effective collaboration and maintain expected productivity levels.
- Review and verify all medications before dispensing to ensure accuracy and safety.
- Keep pharmacy records compliant with applicable laws and regulatory standards.

- Possess in-depth expertise in specialty pharmacy practices.
- Take responsibility for assessing and managing medication therapy for home care patients in coordination with physicians, nurses, and other clinicians.
- Attend regular team meetings to share and receive operational and clinical updates.
- Build and maintain constructive relationships with internal teams and external partners.
- Meet customer service expectations and proactively address client needs.
- Resolve customer concerns fairly and with empathy.
- Collaborate with colleagues in organizing pharmacy operations while safeguarding patient confidentiality.

- Work schedule includes Monday to Friday shifts between 8 AM and 5:30 PM, with three shift options: 8-4:30, 8:30-5, or 9-5:30; one evening shift weekly from 5:30 PM to 8 AM, rotating Monday through Friday; weekend on-call duties required.
- Initial training is conducted on-site, with the option to transition to remote or hybrid work afterward.
- Remote employees must have a cable broadband or fiber optic internet connection with minimum speeds of 10 Mbps download and 5 Mbps upload.
- The employer enforces a tobacco-free policy and may decline to hire tobacco or nicotine users in states where permitted by law, including Pennsylvania.
- Candidates in applicable states who use tobacco or nicotine may still qualify if they enroll in an approved smoking cessation program before employment begins.
