Flagler Health is seeking a dedicated and compassionate Spanish-speaking LPN/LVN to join our team as a remote contractor. In this role, you will deliver Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) services, directly engaging with patients to support their daily improvement.
What You'll Do
- Facilitate patient enrollment into our RTM or CCM programs using Flagler's portal.
- Make daily calls and send text messages to document patient health status and maintain engagement.
- Assist in collecting and documenting patient health information, including histories and new symptoms.
- Support patients by sending health pointers such as stretches, meditation videos, and light exercises.
- Maintain strict confidentiality of patient information, adhering to HIPAA regulations and company policies.
- Participate in ongoing company training and check-ins.
What We're Looking For
- Certification as an LPN.
- 3+ years of experience working in an office or remote setting.
- Proficiency with technology such as Zoom, Slack, and online enrollment platforms.
- Strong organizational skills and the ability to manage multiple tasks efficiently.
- Excellent verbal and written communication skills, with a focus on empathy and patient support.
- Ability to work independently in a remote setting, showing initiative and self-motivation.
- Familiarity with HIPAA regulations and a commitment to maintaining patient privacy.
