Responsibilities
- Foster a welcoming, joyful center environment where participants and team members feel they belong.
- Actively seek and provide feedback to support continuous improvement in clinical outcomes and participant experience.
- Uphold and demonstrate the organization’s core values in daily work.
- Work with the Interdisciplinary Team to enhance participant health and quality of life through integrated care planning.
- Assess participants' psychosocial needs, preferences, and personal goals.
- Deliver and document social work services such as resource coordination, care transitions support, counseling, and case management as outlined in care plans.
- Coordinate with primary care providers and care team members to ensure seamless transitions across care settings.
- Organize and lead care conference meetings involving care team members, participants, and support networks to optimize care coordination.
- Manage discharge planning for participants leaving the program.
- Complete additional duties as assigned.
