About the Role
The position involves delivering clinical social work services on an as-needed basis, conducting assessments, developing care plans, and collaborating with healthcare professionals to support patient well-being.
Responsibilities
- Conduct psychosocial assessments for patients and families
- Develop individualized care plans in collaboration with healthcare providers
- Provide counseling and emotional support to patients coping with illness
- Connect patients with community resources and support services
- Assist in discharge planning to ensure safe transitions of care
- Document patient interactions and progress in medical records
- Advocate for patient needs within the healthcare system
- Participate in care coordination meetings
- Address social determinants of health impacting patient outcomes
- Maintain compliance with confidentiality regulations
- Support patients and families through crisis intervention
- Collaborate with physicians, nurses, and therapists
- Identify barriers to treatment adherence and propose solutions
- Facilitate referrals to mental health programs
- Promote patient autonomy in care decision-making
- Respond to urgent psychosocial needs as they arise
- Uphold professional standards of social work practice
- Engage in culturally sensitive communication
- Assist with advance care planning discussions
- Monitor patient progress and adjust interventions as needed
