Responsibilities

  • Assess clients to identify individual problem areas necessitating treatment
  • Complete assessments
  • Coordinate interdisciplinary team meetings
  • Coordinate discharge planning efforts among the multidisciplinary treatment team
  • Coordinate the overall treatment plan implementation for the client on the unit
  • Work closely with nursing staff to ensure continuity of treatment efforts
