Responsibilities
- Manage high-priority cases with attention to detail and regulatory standards
- Conduct thorough assessments to determine client eligibility and needs
- Develop individualized service plans in collaboration with stakeholders
- Maintain up-to-date case files and documentation
- Coordinate referrals to appropriate support services

- Ensure compliance with organizational and legal requirements
- Respond to inquiries from internal and external parties
- Participate in team meetings and case reviews
- Identify barriers to client success and recommend solutions
