Responsibilities
- Capture detailed patient health histories, including symptoms, past diagnoses, and relevant medical background.
- Document clinical findings, treatment plans, and follow-up directions provided by physicians or healthcare professionals.
- Transcribe spoken medical instructions, including prescriptions and test orders, into accurate written records.
- Maintain and update electronic health records (EHR/EMR) with current and accurate patient data.
- Support communication and coordination among medical staff, patients, and other professionals.
- Ensure all documentation meets accuracy requirements and adheres to institutional and regulatory guidelines.
- Perform additional duties as assigned.
