Position OverviewWe are hiring a remote CCM/RPM Enrollment Coordinator for nationwide service across the United States. This role is ideal for a Certified Medical Assistant passionate about patient education and long-term care support. You will play a key role in connecting patients with essential chronic care services, ensuring they understand their care plans and feel confident in managing their health.
Key Responsibilities
- Reach out to patients to introduce and enroll them in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs
- Determine patient eligibility and clearly communicate the value and structure of each program
- Secure verbal consent in alignment with CMS regulations and accurately record all interactions
- Provide education on chronic conditions, medication use, and care plan expectations to patients and caregivers
- Collaborate with healthcare providers and internal teams to support seamless care coordination
- Deliver compassionate, proactive communication to enhance patient engagement and trust
- Encourage adherence to treatment plans and promote self-care strategies
- Maintain accurate records in compliance with CMS, HIPAA, and CCS standards
Qualifications
Candidates must hold a valid Certified Medical Assistant (CMA or RMA) credential. Experience in chronic care, telehealth, or care coordination is highly valued. Strong communication skills, attention to detail, and familiarity with federal healthcare guidelines are essential.
Work Environment
This is a fully remote position open to applicants anywhere in the U.S. You will work independently with structured support, using secure systems to interact with patients and clinical teams. Flexibility is built into the schedule while maintaining accountability to patient outreach goals.
Compensation
Hourly pay ranges from $17.00 to $20.00, based on experience and qualifications. Additional compensation details will be shared during the interview process.
