Support patients managing chronic conditions from anywhere in this fully remote LPN role. As a Remote Care Coordinator, you’ll conduct monthly check-ins, guide patients through enrollment in care management programs, and perform comprehensive health risk evaluations to support long-term wellness.
What You’ll Do
- Lead monthly telephonic outreach to patients enrolled in Chronic Care Management, ensuring continuity of care
- Complete health assessments and assist with transitional care services for high-risk individuals
- Support Remote Patient Monitoring initiatives by tracking vital data and identifying care gaps
- Educate patients and caregivers on diagnoses, medication regimens, and self-care strategies
- Coordinate with providers and internal teams to align care plans and improve outcomes
- Document all interactions thoroughly in electronic health record systems
- Maintain compliance with HIPAA, CMS, and quality reporting standards
- Balance productivity goals with empathetic, patient-centered communication
What We Require
- Current Licensed Practical Nurse (LPN) credential in good standing
- Experience with EHR platforms and virtual care delivery
- Strong communication skills and a patient-first approach
