About the Role

As a Physician Assistant in Physical Medicine and Rehabilitation, you’ll play a central role in enhancing patient recovery and independence within skilled nursing facilities. You’ll deliver focused medical care to individuals recovering from stroke, neurological conditions, and orthopedic procedures, working closely with physical, occupational, and speech therapy teams to align medical management with functional progress.

Clinical Responsibilities

You will lead clinical coordination at your assigned facility, partnering with primary care providers and interdisciplinary teams to manage complex conditions such as neuropathic pain, spasticity, neurogenic bowel and bladder, and postoperative complications. Your assessments and treatment plans will directly support therapy goals, ensuring patients achieve optimal outcomes.

You’ll attend regular care team meetings, track patient progress through therapy milestones, and help identify and resolve medical barriers to rehabilitation. This role emphasizes clinical leadership, thoughtful collaboration, and continuity across medical and rehabilitative care.

Professional Support and Development

You’ll receive full support through virtual scribe services to reduce documentation burden, allowing greater focus on patient care. Qualifications and Requirements

Licensure as a Physician Assistant in California is required. A strong foundation in post-acute or skilled nursing care is preferred. Candidates must be authorized to work in the United States and demonstrate a consistent commitment to high-quality, compassionate patient care.

Work Schedule and Location

This is an onsite role based in Oakland or San Pablo, CA, with a flexible four-day workweek. Core hours are generally between 9 AM and 4 PM, and your specific workdays (Monday through Friday) can be selected during onboarding to support work-life balance.

Benefits and Compensation

The position offers a competitive salary starting at $125,000, with additional bonus potential.
