Metro Physical & Aquatic Therapy is hiring a Physical Therapist- Homecare Part B (Per Diem)

Responsibilities

  • Provide therapy services to patients within a specified service area
  • Assess patient needs for physical therapy in home environments and establish personalized treatment strategies
  • Collaborate with physicians, patients, and family members to create care plans and set treatment objectives
  • Design, organize, and carry out individualized therapeutic programs aimed at restoring functional abilities and fostering independence
  • Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
  • Ensure timely and precise documentation for every patient visit
  • Maintain clear communication with patients, families, and members of the care team, particularly during discharge planning
  • Deliver services in accordance with agency guidelines and professional standards
