Responsibilities
- Serve patients within an assigned geographic region
- Deliver individualized, one-on-one therapy sessions
- Assess patients and create tailored treatment plans following physician directives and patient-specific needs
- Administer care consistent with professional standards, agency guidelines, and prescribed care plans
- Train family members, home health aides, and other caregivers in appropriate therapy methods
- Design and implement therapy programs to maintain, enhance, or restore physical abilities, reduce pain, and prevent occupational decline
- Conduct and document initial evaluations, analyzing findings to define issues and establish diagnoses before treatment begins
- Ensure timely and accurate recordkeeping for every patient visit
- Monitor treatment outcomes throughout care and modify interventions to optimize results
- Demonstrate willingness to work with older adult patients
