DaVita was looking for a Nurse Practitioner Manger - Remote - Georgia, NC, SC

DaVita is hiring a full-time Nurse Practitioner Manager for a remote role supporting clinical teams in Georgia, North Carolina, and South Carolina. In this leadership position, you will guide Nurse Practitioners to deliver quality, cost-effective care for CKD and ESKD patients, focusing on education, HCC coding, risk adjustment, and population health management to support the program's financial viability.

What You'll Do

  • Serve as the clinical lead for a team of Nurse Practitioners and oversee their work.
  • Ensure NP quality and productivity, including accurate documentation, appropriate diagnoses, closing Plan of Care gaps, and coding.
  • Support NP hiring efforts and oversee the onboarding of new NPs.
  • Provide ongoing clinical guidance and training to improve understanding of CKD, ESKD, and associated chronic conditions.
  • Serve as a key leader of clinical and operational strategy with the Medical Officer and Operations leadership.
  • Identify opportunities to streamline NP workflow and focus management on high-risk, high-utilization members.
  • Lead complex case reviews and provide timely support for escalations involving high-utilization, high-risk members.
  • Know and comply with DaVita policies, procedures, and state and federal regulations.
  • Perform other duties as assigned.

What We're Looking For

  • Demonstrated experience documenting and coding from a Medicare Advantage (MA) risk perspective.
  • Experience conducting Health Risk Assessments on high-risk and chronically ill patients.
  • Documented experience in HCC and Risk Assessments, focusing on chart audits and quality metrics.
  • Residence in Georgia and ability to travel up to 20% to support teams in South Carolina and North Carolina.
  • 4-6 years of Nurse Practitioner experience.
  • Experience in population health, primary/family care, and/or nephrology.
  • Master's degree in Nursing.
  • Current NP License in the state of Georgia and ability to obtain licensure in North Carolina and South Carolina upon hire.
  • Excellent verbal and written communication skills.
  • Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation for chronic disease care.
  • Ability to handle confidential information with discretion and judgment.
  • Intermediate computer skills and proficiency in MS Word, Excel, and Outlook.
  • Excellent organizational skills and attention to detail.

Nice to Have

  • At least 2 years of experience supervising or leading Nurse Practitioners.
  • Demonstrated experience with HCC Coding.

This is a fully remote position requiring residence in Georgia, serving teams across Georgia, North Carolina, and South Carolina.
