About the Role
The position involves coordinating long-term support services for members by assessing needs, creating care plans, and connecting individuals with appropriate resources to improve health outcomes and quality of life.
Responsibilities
- Conduct initial assessments to identify member needs and eligibility for services
- Develop personalized care plans in collaboration with members and providers
- Coordinate access to long-term support services and community resources
- Monitor member progress and adjust care plans as needed
- Maintain accurate and timely documentation in case management systems
- Serve as a liaison between members, families, and healthcare providers
- Ensure compliance with regulatory and program-specific requirements
- Facilitate transitions of care between settings such as home, facility, or hospital
- Educate members on available services and care options
- Advocate for member needs within the healthcare system
- Respond to inquiries from members, families, and providers
- Participate in care coordination meetings and case conferences
- Track service utilization and outcomes for quality improvement
- Support members in achieving personal health and wellness goals
- Promote member independence and community integration
- Identify and address barriers to care access
- Collaborate with interdisciplinary teams to deliver holistic care
- Conduct follow-ups to ensure service delivery and satisfaction
- Assist with crisis intervention when necessary
- Uphold confidentiality and ethical standards in all interactions
Nice to Have
- Master's degree in social work, counseling, or healthcare field
- Certification in care coordination or case management
- Experience with Medicaid or government-funded programs
- Familiarity with LTSS assessment tools
- Bilingual communication skills
