Signify Health, a CVS Health® division, is recruiting a Per Diem Clinician (Nurse Practitioner or Physician Assistant) to conduct In-Home Health Assessments, engaging with people in their personal environments and addressing care continuity gaps. You'll be instrumental in establishing trusted connections to enhance health outcomes, delivering flexible, comprehensive, and personalized health evaluations directly to individuals' homes. Job Highlights The 45-minute visit facilitates one-on-one interactions and health inquiry resolution. It encompasses medication and medical history review, physical examination, and potentially additional diagnostic screenings as specified by healthcare plans. Utilizing an iPad with streamlined clinical workflow, you'll conduct evaluations without prescribing medications or modifying treatment protocols.
