Responsibilities
- Provide services to patients within a specified service region
- Assess patient therapy requirements in home environments and create personalized rehabilitation strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
- Design, organize, and carry out individualized treatment programs aimed at restoring physical function and self-sufficiency
- Educate caregivers, family members, physical therapist assistants, and home health aides on proper therapeutic methods
- Ensure accurate and prompt documentation for every patient encounter
- Maintain clear communication with patients, families, and healthcare team members, particularly during care transition and discharge planning
- Deliver services in accordance with organizational guidelines and professional standards
