Responsibilities
- Provide patient care within a specified local region
- Assess patient therapy needs in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic goals
- Design, organize, and carry out individualized rehabilitation programs to improve function and independence
- Educate caregivers, physical therapist assistants, home health aides, and family on proper therapy methods
- Maintain accurate and prompt documentation for every patient visit
- Communicate clearly with patients, families, and interdisciplinary team members, particularly regarding discharge planning
- Deliver services in alignment with agency guidelines and professional standards
