Responsibilities
- Provide patient care within an assigned geographic region
- Assess patient needs for physical therapy services in home environments and create personalized treatment plans
- Collaborate with physicians, patients, and family members to establish care plans and set treatment objectives
- Design, organize, and carry out individualized therapy programs aimed at restoring functional abilities and fostering independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic techniques
- Maintain accurate and timely documentation for every patient visit
- Communicate clearly with patients, families, and members of the interdisciplinary team, particularly regarding discharge planning
- Deliver services in accordance with agency policies and standards of professional practice
