Metro Physical & Aquatic Therapy is hiring a Home Care Physical Therapist-Per Diem (Manhattan)

Responsibilities

  • Provide patient care within an assigned geographic region
  • Assess patient needs for physical therapy services in home environments and create personalized treatment plans
  • Collaborate with physicians, patients, and family members to establish care plans and set treatment objectives
  • Design, organize, and carry out individualized therapy programs aimed at restoring functional abilities and fostering independence
  • Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic techniques
  • Maintain accurate and timely documentation for every patient visit
  • Communicate clearly with patients, families, and members of the interdisciplinary team, particularly regarding discharge planning
  • Deliver services in accordance with agency policies and standards of professional practice
