Responsibilities
- Provide care to patients within a defined local region
- Assess patient therapy needs in their home environment and create personalized treatment strategies
- Establish individualized care plans and therapy objectives in collaboration with physicians, patients, and family members
- Design and carry out tailored rehabilitation programs to improve function and support independent living
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Maintain accurate and timely records for every patient visit
- Communicate clearly with patients, families, and members of the care team, particularly during transitions of care
- Deliver services in accordance with organizational policies and professional standards
