Responsibilities
- Serve patients within a specified local region
- Assess patient requirements for physical therapy services in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
- Design, organize, and carry out individualized therapy programs aimed at restoring functional abilities and supporting independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Maintain accurate and prompt documentation for every patient visit
- Communicate clearly with patients, families, and members of the interdisciplinary team, particularly regarding discharge coordination
- Deliver services in alignment with organizational policies and professional standards
