Metro Physical & Aquatic Therapy is hiring a Home Care Physical Therapist- Per Diem

Responsibilities

  • Manage and treat patients within a specific geographic area
  • Assess patients' physical therapy needs in a home setting and create tailored treatment plans
  • Collaborate with physicians, patients, and families to establish care plans and treatment objectives
  • Design, prepare, and execute customized therapy programs to enhance function and encourage independence
  • Train family members, PTAs, Home Health Aides, and caregivers in appropriate therapy methods
  • Maintain accurate and timely documentation for each patient visit
  • Communicate effectively with patients, families, and interdisciplinary team members, particularly regarding discharge planning
  • Provide care that adheres to agency policies and professional practice standards
