Responsibilities
- Manage and treat patients within a specific geographic area
- Assess patients' physical therapy needs in a home setting and create tailored treatment plans
- Collaborate with physicians, patients, and families to establish care plans and treatment objectives
- Design, prepare, and execute customized therapy programs to enhance function and encourage independence
- Train family members, PTAs, Home Health Aides, and caregivers in appropriate therapy methods
- Maintain accurate and timely documentation for each patient visit
- Communicate effectively with patients, families, and interdisciplinary team members, particularly regarding discharge planning
- Provide care that adheres to agency policies and professional practice standards
