Metro Physical & Aquatic Therapy is hiring a Home Care Physical Therapist- Per Diem

Responsibilities

  • Serve patients within a specified local region
  • Assess patient therapy needs in home environments and create personalized treatment strategies
  • Establish care plans and therapeutic objectives in collaboration with physicians, patients, and family members
  • Design and carry out individualized rehabilitation programs to improve function and self-sufficiency
  • Educate family members, physical therapy assistants, home health aides, and caregivers on proper therapeutic methods
  • Maintain prompt and precise visit records
  • Exchange clear and timely information with patients, families, and care team members, particularly regarding transition after care
  • Deliver services in alignment with organizational guidelines and professional standards
