Responsibilities
- Serve patients within a specified local region
- Assess patient therapy needs in home environments and create personalized treatment strategies
- Establish care plans and therapeutic objectives in collaboration with physicians, patients, and family members
- Design and carry out individualized rehabilitation programs to improve function and self-sufficiency
- Educate family members, physical therapy assistants, home health aides, and caregivers on proper therapeutic methods
- Maintain prompt and precise visit records
- Exchange clear and timely information with patients, families, and care team members, particularly regarding transition after care
- Deliver services in alignment with organizational guidelines and professional standards
