Responsibilities
- Provide patient care within an assigned geographic region
- Assess patient requirements for physical therapy in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
- Design, organize, and execute individualized therapy programs aimed at restoring functional abilities and fostering independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Ensure accurate and prompt documentation for every patient visit
- Maintain clear communication with patients, families, and interdisciplinary team members, particularly regarding discharge planning
- Deliver services in alignment with agency protocols and professional standards
