Responsibilities
- Provide care to patients within an assigned geographic region
- Assess patient needs for physical therapy services in home environments and create personalized treatment strategies
- Establish individualized plans of care and therapeutic goals in collaboration with physicians, patients, and family members
- Design, organize, and carry out tailored rehabilitation programs aimed at restoring functional ability and fostering independence
- Educate family members, physical therapist assistants, home health aides, and caregivers in proper therapeutic methods
- Ensure accurate and prompt documentation for every patient visit
- Maintain clear communication with patients, families, and members of the interdisciplinary team, particularly during discharge planning
- Deliver services in alignment with agency protocols and standards of professional practice
