Responsibilities
- Accept and manage patient cases within a specified geographic region
- Assess patient requirements for physical therapy in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
- Design, organize, and execute tailored therapy programs aimed at restoring functional abilities and independence
- Train family members, physical therapist assistants, home health aides, and caregivers in proper therapeutic techniques
- Ensure timely and precise documentation for every patient visit
- Maintain clear communication with patients, families, and interdisciplinary team members, particularly during discharge planning
- Deliver services in accordance with agency policies and professional practice guidelines
