Responsibilities
- Accept and treat patients within your designated geographic area
- Evaluate patients’ needs for physical therapy in the home setting and develop individualized treatment plans
- Develop a plan of care and treatment goals in consultation with the physician, patient, and family
- Plan, prepare and implement customized therapy programs to restore function and promote independence
- Instruct family members, PTAs, Home Health Aides, and caregivers on appropriate therapy techniques
- Provide timely and accurate documentation for each visit
- Communicate effectively with patients, families, and interdisciplinary team members, especially around discharge planning
- Deliver care in compliance with agency policies and professional practice standards
Requirements
- Physical Therapist license
- Ability to evaluate patients and develop individualized treatment plans
- Ability to develop plan of care and treatment goals in consultation with physician, patient, and family
- Ability to implement customized therapy programs
- Ability to instruct family members, PTAs, aides, and caregivers
- Provide timely and accurate documentation
- Effective communication with patients, families, and team members
- Deliver care in compliance with professional practice standards
