Responsibilities
- Provide care to patients within a specified geographic region
- Assess patient needs for physical therapy services in home environments and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish plans of care and therapeutic goals
- Design, organize, and carry out individualized rehabilitation programs aimed at restoring functional abilities and fostering independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Ensure accurate and prompt documentation for every patient visit
- Maintain clear communication with patients, families, and members of the interdisciplinary team, particularly regarding discharge planning
- Deliver services in alignment with agency protocols and standards of professional practice
