Responsibilities
- Provide in-home physical therapy services within an assigned service region
- Assess patient conditions and determine appropriate therapy needs in home environments
- Collaborate with physicians, patients, and family members to establish individualized care plans
- Create and oversee personalized rehabilitation programs aimed at restoring functional ability
- Educate caregivers, physical therapist assistants, and home health aides on therapeutic methods
- Maintain precise and prompt documentation for all patient visits
- Maintain clear communication with patients, families, and care team members, particularly during care transitions
- Deliver services in alignment with organizational policies and clinical standards
