Responsibilities
- Provide care to patients within an assigned service area
- Assess patient requirements for physical therapy in home environments and establish individualized treatment strategies
- Collaborate with physicians, patients, and family members to design care plans and set therapeutic goals
- Create, organize, and execute personalized rehabilitation programs aimed at restoring mobility and independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapy methods
- Ensure accurate and prompt documentation for every patient visit
- Maintain clear communication with patients, families, and interdisciplinary team members, particularly during discharge planning
- Deliver services in accordance with agency protocols and professional standards
