Metro Physical & Aquatic Therapy is hiring a Home Care Physical Therapist- Full Time

Responsibilities

  • Provide therapy services to patients within a defined local region
  • Assess patient requirements for physical therapy in home environments and create personalized treatment strategies
  • Collaborate with physicians, patients, and family members to establish care plans and set therapy objectives
  • Design, organize, and carry out individualized rehabilitation programs aimed at restoring functional abilities and encouraging self-sufficiency
  • Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
  • Ensure prompt and precise documentation for every patient visit
  • Maintain clear communication with patients, families, and members of the interdisciplinary team, particularly during discharge planning
  • Deliver services in accordance with agency guidelines and professional standards
