Responsibilities
- Manage and treat patients within a specific geographic region
- Assess patients' physical therapy needs at home and create personalized treatment plans
- Collaborate with physicians, patients, and families to establish care plans and treatment objectives
- Design, prepare, and execute tailored therapy programs to enhance function and encourage self-reliance
- Train family members, physical therapist assistants, home health aides, and caregivers in appropriate therapy methods
- Maintain accurate and timely documentation for each visit
- Communicate effectively with patients, families, and the interdisciplinary team, with a focus on discharge planning
- Provide care that adheres to agency policies and professional standards
