Responsibilities
- Provide care to patients within a specified local region
- Assess therapy needs of patients in their home environment and create personalized treatment strategies
- Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
- Design, organize, and carry out individualized rehabilitation programs aimed at restoring mobility and independence
- Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
- Ensure accurate and prompt documentation for every patient visit
- Maintain clear communication with patients, families, and other healthcare providers, particularly during care transition planning
- Deliver services in alignment with organizational guidelines and professional standards
