Metro Physical & Aquatic Therapy is hiring a Home Care Physical Therapist-1099 (Queens)

Responsibilities

  • Provide care to patients within a specified local region
  • Assess therapy needs of patients in their home environment and create personalized treatment strategies
  • Collaborate with physicians, patients, and family members to establish care plans and therapeutic objectives
  • Design, organize, and carry out individualized rehabilitation programs aimed at restoring mobility and independence
  • Educate family members, physical therapist assistants, home health aides, and caregivers on proper therapeutic methods
  • Ensure accurate and prompt documentation for every patient visit
  • Maintain clear communication with patients, families, and other healthcare providers, particularly during care transition planning
  • Deliver services in alignment with organizational guidelines and professional standards
