Metro Physical & Aquatic Therapy is hiring a Home Care Occupational Therapist- Per Diem (South Shore, Nassau County)

Requirements

  • Serve patients within a specified geographic region
  • Deliver one-on-one therapy sessions
  • Assess patients and create personalized treatment plans in accordance with physician directives and individual needs
  • Administer care consistent with professional standards, agency guidelines, and prescribed care plans
  • Instruct family members, home health aides, and other caregivers in appropriate therapeutic techniques
  • Design and implement occupational therapy programs to preserve, enhance, or restore physical function, reduce pain, and prevent loss of daily living skills
  • Conduct and document initial evaluations, analyzing findings to define issues and establish a diagnosis before treatment begins
  • Maintain accurate and timely records for every patient visit
  • Monitor treatment outcomes at various intervals and modify interventions to ensure optimal results
  • Willingness to work with elderly patients
