Metro Physical & Aquatic Therapy is hiring a Home Care Occupational Therapist- Per Diem

Responsibilities

  • Assess and treat patients within an assigned geographic region
  • Deliver individualized, one-on-one therapy sessions
  • Conduct patient evaluations and create customized treatment plans following physician directives and patient-specific needs
  • Administer care consistent with professional guidelines, agency protocols, and prescribed care plans
  • Instruct family members, home health aides, and caregivers in appropriate therapeutic techniques
  • Design and implement occupational therapy programs to maintain, enhance, or restore physical abilities, reduce pain, and prevent functional decline
  • Complete and document initial patient evaluations, analyze findings, formulate diagnoses, and plan interventions
  • Ensure accurate and prompt documentation for every patient visit
  • Monitor treatment outcomes throughout the care process and modify interventions to optimize results
  • Demonstrate willingness to work with older adult patients
