Habitat Health is hiring a Home Care Coordinator

Responsibilities

  • Demonstrate alignment with organizational values in all interactions and responsibilities.
  • Carry out detailed in-home evaluations to determine care requirements, functional capabilities, safety concerns, and suitability for home care services.
  • Assess home environments for safety hazards and recommend modifications to support independence and reduce risks.
  • Evaluate participants' ability to perform daily tasks and use assistive devices through observation and return demonstrations.
  • Engage with participants, families, and caregivers to identify care needs and areas where services may be lacking.
  • Actively contribute to the Interdisciplinary Team in creating, executing, and revising individualized care plans.
  • Work collaboratively with clinical and non-clinical staff, including rehabilitation, nutrition, and social services, to gain a full understanding of participant needs.
  • Coordinate home care services in accordance with personalized care plans and participant goals, and review caregiver task completion to identify missed or declined services.
  • Review weekly home care documentation to detect changes in condition, service gaps, Service Determination Requests, grievances, or issues needing escalation.
  • Keep service authorizations current and accurate, ensuring alignment with participant needs and payer guidelines.
  • Track authorization timelines and manage renewals to avoid service disruptions.
  • Ensure timely, precise, and compliant recordkeeping in electronic health and operational systems.
  • Support quality assurance and compliance efforts related to home care delivery.
  • Complete additional duties as assigned by leadership.

  • Work in healthcare, administrative, community, or remote settings based on role requirements.
  • Communicate clearly with participants, caregivers, colleagues, and stakeholders via in-person, phone, and digital channels.
  • Remain stationary or move throughout environments for extended durations as required by duties.
  • Operate standard office and clinical equipment such as computers, phones, medical devices, and documentation systems.
  • Travel between facilities or community locations as necessary.
  • Follow safety protocols, infection control practices, and use personal protective equipment when required.
  • Perform physical tasks including bending, reaching, lifting, or assisting participants, depending on role scope.
  • Lift, carry, push, or pull items typically weighing 25–50 pounds, with assistance or mechanical support as needed.
  • Employment is at-will and may be terminated by either party at any time.
  • Team members in care settings must provide proof of vaccination or a signed declination and follow masking rules as required by law.
  • The organization participates in E-Verify and will submit Form I-9 information to confirm work authorization in the U.S.
