Position Summary
Candidate must support 8:00 AM - 5:00 PM Mountain Standard Time (MST) or 7:00 AM - 4:00 PM Pacific Standard Time (PST).
Aetna's Healthy Aging Support Program is a short-term, self-management education program. The Health Coach provides information, referral, assistance with accessing resources, health coaching, and supportive care to Medicare members. You'll instruct group coaching classes on specific content addressing common health concerns for members with chronic conditions, providing education and resources, and effectively utilizing community, healthcare, and plan benefit resources.
Candidates with experience in care management, healthcare, social work, aging network, or similar backgrounds are encouraged to apply!
Additional responsibilities include but are not limited to:
- Educate members on topics like Managing Medicines, Nutrition, Safety, Benefits/Resources, Mindfulness, and Working with Care Team, empowering group coaching participants
- Assist members in actively participating with healthcare providers and Aetna Care Management teams regarding decisions impacting their health and well-being
- Document all member interactions in the appropriate member record(s)
- Demonstrate empathy for situations faced by our members and their caregivers
- Troubleshoot unavailable resources, securing support to close member gaps
- Follow program workflows consistently, following up with members to review personal action plans and determine additional referrals (e.g., Care Management, Behavioral Health)
Required Qualifications
- At least 2 years working as a Health Coach
- Health Coach certification required
- Candidate must support 8:00 AM - 5:00 PM MST or 7:00 AM - 4:00 PM PST
Preferred Qualifications
- Community health or public health experience preferred
- Managed Care/Medicare/Medicaid experience preferred
- Bilingual preferred
- Bachelor's degree preferred
Education
Associate's degree or equivalent combination of education and experience

Pay Range
The typical pay range for this role is: $21.10 - $49.08
This pay range represents the base hourly rate or base annual full-time salary for positions in this job grade. The actual base salary offer depends on factors including experience, education, geography, and other relevant considerations. This position is eligible for a CVS Health bonus, commission, or short-term incentive program in addition to the base pay range.
