Responsibilities
- Collaborate with the SVP to manage end-to-end onboarding and integration of new partners, including those acquired through mergers, acquisitions, or Medicare Advantage expansions.
- Align partner workflows with dementia care operations, ensuring adherence to internal performance and compliance benchmarks.
- Drive expansion initiatives within established partnerships, identifying opportunities in new markets or service offerings post-integration.
- Act as the main point of contact and relationship lead for high-priority network partners.
- Track partner performance metrics and deliver proactive support to ensure success in dementia clinical programs.
- Implement and manage clinical programs and value-based care models to achieve top-tier patient outcomes.
- Build strong operational knowledge to train and coach internal teams, while identifying process improvements for greater efficiency.
- Oversee reporting, billing, compliance monitoring, and partner-specific invoicing and operational summaries.
- Coordinate with Clinical Operations, Business Development, Growth, and Finance teams to maintain accurate documentation and performance insights.
- Contribute to building dashboards and analytical tools that guide business strategy and internal decision-making.
- Help develop leadership presentations, external materials, and lead internal performance reviews.
- Partner with clinical, care navigation, finance, and business development teams to integrate partner feedback into service design and innovation.
Compensation
Pay Range for Posted Region $120,000 - $135,000 USD
