SynaptiCure Inc. is hiring a Dementia Care Nurse Practitioner

Responsibilities

  • Assess and manage the health of patients with dementia.
  • Develop and implement care plans tailored to individual patient needs.
  • Administer medications and treatments as prescribed.
  • Monitor patient progress and adjust care plans as necessary.
  • Collaborate with interdisciplinary teams to ensure comprehensive care.
  • Educate patients and families about dementia and its management.
  • Provide emotional support to patients and their families.
  • Coordinate care with other healthcare providers.
  • Document patient care and maintain accurate records.
  • Participate in ongoing education and training to stay current with best practices.
  • Advocate for patients' rights and well-being.
  • Ensure a safe and comfortable environment for patients.
  • Conduct regular check-ups and assessments.
  • Manage patient symptoms and behaviors.
  • Facilitate communication between patients, families, and healthcare team.
  • Promote patient independence and quality of life.
  • Implement non-pharmacological interventions as needed.
  • Provide end-of-life care and support.
  • Participate in research and quality improvement initiatives.
  • Maintain confidentiality and professionalism in all interactions.

Nice to Have

Education

  • Master's degree in Nursing from an accredited program.
  • Completion of a Nurse Practitioner program with a focus on geriatrics or dementia care.

Certifications

  • Board certification in Gerontological Nursing (BC-GNP) or equivalent.
  • Certification in Dementia Care (CDC) or equivalent.

Experience

  • Minimum of 2 years of experience in a clinical setting.
  • Experience working with patients with dementia or other cognitive impairments.

Skills

  • Strong clinical assessment and diagnostic skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.

Licensure

  • Current and valid Nurse Practitioner license in the state of practice.
  • Board certification in a relevant specialty.

Training

  • Completion of a Nurse Practitioner program with a focus on geriatrics or dementia care.
  • Ongoing education and training in dementia care and management.

Knowledge

  • Knowledge of dementia management strategies.
  • Knowledge of current healthcare regulations and standards.
