About the Role
The Clinical Care Manager will oversee patient care coordination, support treatment adherence, and collaborate with healthcare providers to improve outcomes for patients with chronic kidney conditions.
Responsibilities
- Manage care plans for patients with chronic kidney disease
- Conduct regular patient assessments and follow-ups
- Collaborate with nephrologists and primary care providers
- Educate patients on treatment protocols and lifestyle adjustments
- Monitor patient progress and adjust care strategies
- Ensure compliance with clinical guidelines and protocols
- Document patient interactions and outcomes accurately
- Support transitions of care between settings
- Respond to patient inquiries and urgent concerns
- Participate in quality improvement initiatives
- Coordinate with dialysis centers and pharmacies
- Track clinical metrics and report trends
- Assist in onboarding new patients to care programs
- Facilitate communication among care team members
- Maintain up-to-date patient records
- Promote patient engagement and retention
- Identify high-risk patients for early intervention
- Support telehealth visits and remote monitoring
- Ensure adherence to privacy and regulatory standards
- Contribute to clinical training materials
Nice to Have
- Experience working with home dialysis patients
- Knowledge of remote patient monitoring tools
- Previous work in a virtual care setting
- Certified Nephrology Nurse (CNN) credential
- Training in motivational interviewing
- Familiarity with care management software
- Background in case management
