Responsibilities

  • Coordinate care plans for patients with complex health needs.
  • Collaborate with healthcare providers to ensure patient care is comprehensive.
  • Monitor patient progress and adjust care plans as necessary.
  • Educate patients and families on health management strategies.
  • Facilitate communication between patients and healthcare team.
  • Document patient interactions and care plans in electronic health records.
  • Assist in the development of care management protocols.
  • Provide support to patients during transitions of care.
  • Conduct regular check-ins with patients to assess their health status.
  • Identify and address barriers to patient care.
  • Participate in interdisciplinary team meetings to discuss patient care.
  • Ensure compliance with regulatory requirements and standards of care.
  • Provide emotional support and encouragement to patients.
  • Coordinate referrals to community resources and support services.
  • Assist in the development of patient education materials.
  • Maintain accurate and up-to-date patient records.
  • Provide feedback to the care team on patient progress and outcomes.
  • Assist in the development of care management programs.
  • Participate in quality improvement initiatives.
  • Provide support to patients and families during difficult times.

Nice to Have

  • Master's degree in nursing, social work, or a related field.
  • Experience in remote care management.
  • Certification in care management or a related field.
  • Experience with electronic health records.
